Local NY Connects Programs

NY Connects: Choices for Long Term Care is a free information and assistance service that is available to older adults, individuals with disabilities, and their families to help make informed decisions about long term services and support options. NY Connects can help you to identify the right type of care and learn more about services and supports that meet your needs, whether you are paying for services yourself, through insurance, or eligible for a government program.
