Case/Care Management Services

  • Description:

    Program coordinates and links senior citizens with community services as needed. Case managers assess clients in their homes to determine their long-term care needs. Clients may be eligible for homecare, respite on a cost-share basis, emergency response systems, home delivered meals or other community-based services, information for caregivers. There is no charge for case management services. Designated parking, ramp, elevator with audible signals and braille controls
