EISEP is a New York State Office for the Aging program, administered by CCFCS, designed to assist older persons who need minimal assistance with housekeeping and personal care. The EISEP Community Collaboration has case managers at four different sites - Catholic Charities Family and Community Services (CCFCS), Urban League of Rochester, Lifespan, and Jewish Family Service.
Services include:
Case management (in-home assessment, linkage to appropriate services, monitoring of services), in home personal care (bathing/dressing), housekeeping (light cleaning/laundry/meal preparation). Non-medical home support services.Non-institutional respite care (EISEP contracts with four social adult day programs to provide services to eligible participants)
