Sullivan NY Connects offers information and assistance to consumers, caregivers, and professionals regarding long term services and supports for the elderly and for children and adults with disabilities. Sullivan NY Connects helps people make informed decisions about long term services and supports by providing comprehensive and unbiased information about the long term care options in Sullivan County.
The Long Term Care Council, advisory board for Sullivan NY Connects, meets once a month. One of its duties is to identify and analyze community needs and to develop strategies to respond to those needs. Members on the Long Term Care Council represent the county's consumer demographics, providers of long term care services, and advocates for long term care. Meetings are open to the public.
