Case Management is a service which is designed to make the service delivery system more accessible and responsive to the individual needs of clients. Case management provides continuity of care as client's needs change. It serves as the focal point for identifying the needs of the clients and to organize service delivery for an individual. Case management is the link between the client, informal caregivers and formal service providers which facilitates an appropriate, comprehensive and coordinated response to the client's needs. The OFA case manager assigned to you serves a variety of functions depending on your needs. These include assessment of need, comprehensive care planning, determination of eligibility, arranging and authorizing services, follow up on service delivery, client monitoring, reassessment of services and discharge planning. Two programs offered through case management are : EISEP-The Expanded In-Home Services for the Elderly Program provides appropriate and cost effective non-medical support services for older persons who are functionally impaired. CSE-Community Services for the Elderly Program is a medical program which provides Home Health Aide services on a limited basis to elderly persons who are not able to care for themselves due to illness or functional impairment. A community Health nurse must visit the home to evaluate the need for the home health aide service and to design a care plan.
