NY Connects is a statewide, free, locally based point of entry service giving unbiased information and assistance. NY Connects can work with anyone who needs information on long term services and supports such as home care, transportation, and meals. Adults or children with disabilities, older adults, family members, caregivers, friends or neighbors, veterans, and helping professionals. The goal is to help individuals remain in their homes as long as possible. NY Connects is an Information and Referral Program for people of all ages.
