In 2011, our four family health centers at Clayton, Watertown, LeRay and Lacona were all recognized for achieving Patient Centered Medical Home Level 3 Designation by the National Committee for Quality PhysRecog MedHome RGB lowresAssurance. This is the highest level of accreditation.
Our largest Family Health Center is conveniently located in Watertown, New York, the Plaza Family Health Center. This center is accepting new patients at this time and accepts many forms of insurance including Medicaid. This center offers family practice (or primary care), dermatology, plastic surgery, and infectious disease care. Samaritan Family Health Network has five additional locations to meet the healthcare needs of outlying rural areas of Cape Vincent, Clayton, LeRay, Lacona and Sackets Harbor.
