Skilled Home Care is when VA Clinician identifies a medical need for daily skilled care at home, service is contracted out to a licensed homecare agency. Often this is a short-term, transitional basis like returning home after hospitalization, wound care, or patient-teaching with new treatments (feeding tubes).
Home Based Primary Care (HBPC) serves chronically ill, essentially home-bound Veterans. An initial in-home assessment is completed and then a plan of care developed. VA providers come to the home including; primary care physician, nurse, therapist, dietician and social work. Visits range from weekly, monthly to quarterly depending on needs identified.
Homemaker – Home Health Aide Program is provided through VA contracts with local licensed homecare agencies to deliver personal care. In-home visits average an hour each and are scheduled 3 to 4 times per week.
